Ha, it's not, I cheated. If you are on the Update Page at LiveJournal.com (or if you're using certain clients, like the most recent Semagic), you can select an icon as normal and enter a word in the 'Other' field, and it will put the chosen icon up with your custom word. In this case, obviously, I used the Sick LJ-mood icon with my custom 'miserablesick.'
